Medication at School

            If you would like your child to be given medication at school (including ALL over the counter products such as Tylenol, Advil, Benadryl or cough drops and including vitamin/herb supplements) Colorado state law mandates the health office must have all of the following:

·        Written parent permission

·        Written physician permission and instructions

·        The medication provided in the original labeled container

For prescription medication, please ask the pharmacist for a second pill bottle for school.  Students are not allowed to carry medication in their pockets.  All medication must be kept in a locked cabinet in the health office.  NEW MEDICATION PERMISSION FORMS ARE NEEDED EACH YEAR for all medications.  Last year’s are not valid.

 

Overnight Field Trips

            If your child needs prescription medication, over the counter medication, or vitamin/herb supplements to be administered by teachers, we are required to comply with the state mandate listed above. Permission forms are available in the health office or from team teachers.

 

Immunization

            Please provide the health office with verification of all immunizations that students received through the school year.  Verification may be faxed to 385-1191.  If a child does not have all the required immunizations, the parent/guardian has fourteen days to get the required immunization.

 

New Vaccine Requirement Affecting Miller 6th Grade

            “Tdap” the tetnus, diphtheria, and pertussis (whooping cough) will be required for students entering 6th grade in 2007 unless they have has a tetanus shot within the last two years.

            Immunization Clinics are held at San Juan Basin Health every Monday.  Call 247-5702 for an appointment.
